WORK PERMIT FORMS

ST. EDWARD HIGH SCHOOL INFORMATION ON ISSUANCE OF WORK PERMITS

The student must have a job before he may apply for the permit. There are three sections to be filled out: 

​

  1. APPLICATION FOR MINOR WORK PERMIT - To be filled out and signed by the parent or guardian. 

  2. PLEDGE OF EMPLOYER - To be filled out by the employer. PLEASE NOTE: Employer's Tax ID number MUST BE filled in. ALSO, Number of Days, Hours Per Day, Starting Time and Quitting Time must be filled in. (These fields must all contain numbers). The words VARIED or DEPENDING in the days, hours or starting/quitting time are not acceptable. 

  3. PHYSICIAN'S CERTIFICATE FOR MINOR WORK PERMIT - To be filled out by a physician or a licensed physician assistant. If you've had a physical for any school sport within the past year, get a copy from the athletic office.

When all three sections have been completed, the student can do one of the following for issuance of the work permit: 

  • Completed applications can be brought to Mrs. Jackie Pieffer in the Palisin Commons, Mrs. Weaver in the Counseling Office, or Mrs. Mo Loesch in the Admin Hallway.

  • Or email the completed application to workpermits@sehs.net. Once processed, the work permit will be emailed back to you.

The information from this application is entered into the State of Ohio's website for Minor Work Permits.

PROOF OF BIRTH (copy of birth certificate) MUST accompany the application.
